Refund policy
Effective Date: June 1, 2026
This policy applies to all purchases from Calibrate IV, including appointments, mobile and in-home services, consultations, memberships, treatment programs, laboratory services, prescription products, patient-specific products, medications, home kits, shipped products, regenerative wellness products, and regenerative wellness services, in every market where Calibrate IV operates.
By completing a purchase, booking an appointment, signing an agreement, or authorizing Calibrate IV to order, prepare, reserve, ship, or coordinate products or services, the customer affirmatively acknowledges and agrees to this policy. This acknowledgment is captured at the time of purchase through a required checkout confirmation, intake signature, or equivalent affirmative action, and is retained by Calibrate IV as part of the order record.
Nothing in this policy limits any consumer right or remedy that cannot legally be waived. Where the law of the state in which a purchase is made requires a specific disclosure, notice, or acknowledgment beyond what is stated here, that additional requirement will be presented to the customer at checkout or intake for that transaction and will govern to the extent of any conflict with this general policy.
1. All Sales Are Final
All sales are final. All payments made to Calibrate IV are nonrefundable, except where a refund is expressly required by applicable law.
This includes deposits, installment payments, membership fees, consultation fees, laboratory fees, provider fees, shipping charges, prescription products, patient-specific products, home kits, treatment programs, regenerative wellness products, and scheduled services.
Payments will not be refunded because of:
- Appointment cancellation or failure to attend
- A change of mind or personal circumstances
- Scheduling conflicts or delays
- Failure to complete a treatment program
- Dissatisfaction with a clinical response or outcome
- Failure to follow treatment, storage, or usage instructions
- Medical ineligibility discovered after clinical services have been performed or patient-specific products have been ordered
- Refusal or failure to accept, retrieve, use, or complete delivered products or scheduled services
- Products being retained, opened, used, punctured, reconstituted, administered, or stored outside Calibrate IV's custody
- A request to cancel after products, services, providers, laboratory work, pharmacy work, supplies, or fulfillment resources have been ordered, reserved, prepared, assigned, or committed
If Calibrate IV confirms that a fulfillment issue occurred, Calibrate IV may, at its discretion and where legally permitted, provide a replacement, reshipment, rescheduled service, or nontransferable account credit. These remedies do not create a right to a monetary refund.
2. Standard IV and In-Home Appointments
Customers should provide at least 24 hours' notice when requesting to cancel or reschedule an appointment.
A timely cancellation may qualify for one rescheduled appointment or a nontransferable account credit at Calibrate IV's discretion. It does not qualify for a monetary refund.
Appointments canceled with less than 24 hours' notice, missed appointments, and appointments canceled after a provider has been assigned or dispatched are nonrefundable and may be subject to a $100 cancellation or deployment fee.
For group appointments, the person making the reservation is responsible for the services and applicable fees associated with every participant included in the booking.
3. Provider Deployment and Attempted Services
A provider deployment fee of $100 may apply when a provider has been assigned or dispatched but treatment cannot be completed because:
- The patient is medically ineligible based on screening, vital signs, medical history, or provider judgment
- Relevant medical information was not disclosed before the appointment
- The environment is unsafe or unsanitary
- The patient declines treatment after the provider arrives
- The patient cannot tolerate or chooses not to complete the procedure
- Adequate venous access cannot be obtained after reasonable attempts
- Treatment cannot be completed because of scheduling or time limitations created by the patient
If treatment is stopped, modified, or declined for safety reasons, the patient remains responsible for provider time, travel, supplies used, products opened or prepared, and other nonrecoverable expenses.
4. Medical Eligibility and Provider Approval
All medical services, prescriptions, and treatment programs are subject to clinical evaluation, medical eligibility, and provider approval.
Completing a purchase does not guarantee that a prescription will be issued or that a particular treatment will be medically appropriate.
If a patient is determined to be medically ineligible, Calibrate IV may provide a nontransferable account credit for any eligible unused portion of the purchase. Completed consultations, laboratory services, provider evaluations, administrative services, and patient-specific products already ordered, prepared, or reserved remain final and nonrefundable.
5. Personalized and Prescription Products
Prescription medications, compounded preparations, injectable products, peptides, hormone therapies, customized vitamin preparations, and other patient-specific products are ordered, prepared, or reserved for an individual patient.
Once an order has been submitted to a pharmacy, laboratory, medical supplier, manufacturer, or fulfillment partner, the purchase is final and nonrefundable.
For health and safety reasons, prescription products and patient-specific products cannot be restocked, resold, exchanged, or accepted for return after leaving the dispensing or fulfillment facility.
6. Home Kits and Shipped Products
Home kits, medications, biological products, temperature-sensitive products, and associated supplies are final sale and nonreturnable once ordered or submitted for fulfillment.
Customers must inspect shipments promptly and notify Calibrate IV in writing of any missing, incorrect, damaged, or potentially compromised shipment within 48 hours of delivery.
The notification must include:
- The customer's full name and order number
- Photographs of the exterior packaging
- A photograph of the shipping label
- Photographs of the product and all contents received
- Photographs showing any visible damage
- A photograph of any temperature indicator included
- A written description of the concern
Calibrate IV will evaluate the documentation and may coordinate a replacement, reshipment, supplier claim, or nontransferable account credit. A fulfillment issue does not automatically create a right to a monetary refund.
Customers must not mail medications, biological materials, opened products, used vials, or temperature-sensitive products back to Calibrate IV unless written return instructions are provided.
7. Delivery and Customer Responsibilities
Customers are responsible for providing a complete and accurate delivery address, telephone number, email address, and any necessary delivery instructions.
An order is considered delivered when the shipping carrier records delivery to:
- The address provided by the customer
- An authorized recipient at that address
- An authorized pickup location
- Another location specifically authorized by the customer
Calibrate IV is not responsible for:
- Incorrect or incomplete addresses supplied by the customer
- Failure to retrieve a delivered package promptly
- Theft, loss, or damage occurring after documented delivery
- Product damage caused by failure to follow storage instructions
- Carrier delays caused by weather, emergencies, service interruptions, or circumstances outside Calibrate IV's reasonable control
Customers must contact Calibrate IV promptly so that available carrier claims, supplier claims, replacement options, or other remedies can be evaluated.
8. Regenerative Wellness Products and Services
Regenerative wellness products and services, including exosome-related and stem-cell-related offerings, may require specialized ordering, clinical coordination, provider scheduling, supplier commitments, temperature-controlled shipping, and products reserved for an individual client.
All regenerative wellness purchases are final and nonrefundable once purchased, except where a refund is expressly required by applicable law.
This includes situations where:
- A specialized product has been ordered, allocated, prepared, or reserved
- An order has been submitted to a laboratory, supplier, pharmacy, or fulfillment partner
- A provider has been assigned or dispatched
- A treatment appointment has been confirmed
- A product has been shipped or delivered
- Any portion of the product or service has been administered, performed, or otherwise provided
- The customer retains or uses the product
- The customer does not achieve the result they expected or desired
Clinical responses and outcomes vary. Payment is for the products, professional time, coordination, and services described in the order. Payment is not conditioned upon achieving a specific clinical result, and no particular outcome is promised or guaranteed.
If Calibrate IV confirms a fulfillment error, Calibrate IV may provide a replacement, reshipment, rescheduled service, or nontransferable account credit.
9. Treatment Completion and Retained Products
A service is considered provided when the scheduled treatment has been administered or when the professional services described in the order have been performed.
A shipped product is considered supplied when carrier records establish delivery to the address authorized by the customer.
Products that have been delivered, retained, opened, used, punctured, reconstituted, administered, or stored outside Calibrate IV's custody are not eligible for return, exchange, or refund.
Dissatisfaction with a clinical outcome does not, by itself, establish that a product or service was defective or materially different from its written description.
10. Memberships, Packages, and Treatment Programs
Membership fees, package payments, and treatment-program payments are final and nonrefundable.
Unless otherwise stated in a separate signed agreement:
- Completed billing periods are final and nonrefundable
- Used treatments, shipped products, provider visits, consultations, and clinical services are nonrefundable
- Cancellation applies only to future billing periods
- Cancellation does not reverse previously authorized charges
- Unused benefits have no refundable cash value
- Products ordered, prepared, reserved, or shipped before cancellation remain chargeable
- Promotional, complimentary, or bonus services have no cash value
Any separate signed membership, treatment-program, financing, or payment-plan agreement will control if it conflicts with this general policy.
11. Deposits and Payment Plans
All deposits and installment payments are final and nonrefundable.
Deposits and installment payments may be applied toward products, provider availability, clinical coordination, laboratory services, fulfillment costs, administrative services, or treatment capacity reserved for the patient.
Canceling a treatment or payment plan does not eliminate amounts already earned, committed to third parties, ordered, prepared, reserved, shipped, delivered, or provided.
The patient remains responsible for amounts due under any signed payment-plan or financing agreement, subject to applicable law.
12. Customer Concerns and Resolution Process
Customers who believe an order is missing, damaged, incorrect, defective, or materially different from the written order confirmation must contact Calibrate IV promptly.
The customer should provide:
- Their full name
- Order number
- Date of purchase
- A detailed description of the concern
- Photographs and supporting records
- The requested resolution
Calibrate IV will review the matter in good faith and may request additional information reasonably necessary to investigate the concern.
Customers are encouraged to provide Calibrate IV with a reasonable opportunity to investigate and resolve concerns before initiating a payment dispute. Nothing in this section waives any right that cannot legally be waived.
13. Payment Disputes
If a payment dispute is initiated, Calibrate IV may provide the payment processor, card network, issuing bank, insurer, legal counsel, or another authorized party with records reasonably necessary to document the transaction.
Records may include:
- Order confirmations
- Itemized invoices and receipts
- Payment authorizations
- Signed agreements and accepted policies, including the checkout or intake record reflecting the customer's affirmative acknowledgment of this policy
- Customer communications
- Appointment and service records
- Fulfillment and supplier records
- Shipping and tracking information
- Proof of delivery
- Confirmation that products or services were retained, administered, or provided
Any health information disclosed for payment or dispute-resolution purposes will be limited to the information reasonably necessary and handled according to applicable privacy requirements.
A payment dispute does not automatically cancel a valid contractual balance or change the final-sale status of products or services that were authorized, ordered, prepared, shipped, delivered, retained, administered, or provided.
Calibrate IV reserves all lawful contractual, collection, and legal remedies.
14. Account Credits, Replacements, and Rescheduling
Any account credit, replacement, reshipment, or rescheduled service offered by Calibrate IV is a discretionary customer-service accommodation unless otherwise required by law.
Unless otherwise stated in writing:
- Account credits are nontransferable
- Account credits have no cash value
- Account credits cannot be exchanged for a refund
- Promotional and complimentary services have no cash value
- Replacement products remain subject to the terms of this policy
- Rescheduled appointments remain subject to availability and cancellation requirements
15. Policy Version and Updates
The policy in effect on the date of purchase applies to that transaction.
Updates apply prospectively and do not modify the terms governing an earlier purchase.
If a separate signed agreement contains terms that conflict with this policy, the separate agreement will control for that specific transaction.
