A more recent 2026 systematic review and network meta-analysis in Frontiers in Nutrition found that NAD+ precursor supplementation has a "selective, domain-specific" effect on fatigue — more consistently helpful for endurance-related fatigue than as a universal energy fix. In other words: promising, but not a guaranteed light-switch effect for everyone.

Cellular Repair: Strong Mechanism, Early-Stage Human Data

NAD+ is a required cofactor for sirtuins and DNA repair enzymes, and preclinical research links higher NAD+ availability to improved mitochondrial function and genomic stability. Much of this evidence currently comes from cell and animal models rather than large human outcome trials, so we describe this benefit as supportive of the body's own repair processes — not a treatment for any specific condition.

Recovery and Physical Performance

This is where the human data gets more specific. A randomized, double-blind trial in amateur runners found that supplementation with NMN (an NAD+ precursor) alongside training improved aerobic capacity over six weeks. Separately, a randomized placebo-controlled trial in elderly adults found that NR combined with pterostilbene supported skeletal muscle recovery after an induced injury. And a 12-week randomized, double-blind, placebo-controlled trial of 108 older adults published in Nutrients found measurable improvements in physical performance and total fatigue scores with NMN supplementation, particularly when taken in the afternoon.

Sleep: A Narrower, More Honest Claim

That same 108-person Nutrients trial is the most relevant human data we have on NAD+ and sleep. Over 12 weeks, participants taking NMN didn't show a significant change in their overall Pittsburgh Sleep Quality Index score — but they did show significant improvements in specific sleep measures, including sleep duration, sleep disturbance, and daytime dysfunction, along with reduced drowsiness. It's a real signal, just not a blanket "cures insomnia" story — and it's exactly the kind of nuance we think matters.

A Note on the Evidence: Oral Precursors vs. IV Delivery

Worth being upfront about: the majority of the randomized controlled trials above studied oral NAD+ precursors (NMN and NR), not intravenous NAD+ itself. Large-scale outcome trials specific to IV-administered NAD+ are still limited in the published literature.
