The Testosterone Talk: Separating Hype from Hard Evidence

Low energy. Stalled progress in the gym. A mental fog that coffee can't fix. For a growing number of men, these aren't just "getting older" — they're signs that testosterone levels have drifted below optimal range. Testosterone therapy gets a lot of hype online — and a lot of fear-mongering, too. Here's what the actual clinical research says, minus the noise.

What the Research Says About Benefits

A systematic review and meta-analysis of randomized, placebo-controlled trials, published in the Journal of Clinical Endocrinology & Metabolism, pooled data from four large RCTs (human clinical trials) totaling 1,779 hypogonadal men. Compared with placebo, TRT produced statistically significant improvements in sexual desire, erectile function, and sexual satisfaction.

The largest and most rigorous data set to date comes from the TRAVERSE trial (human clinical, randomized, double-blind, placebo-controlled, NEJM 2023), which enrolled 5,246 men aged 45–80 with confirmed hypogonadism. A nested Sexual Function Study within TRAVERSE found that two years of TRT improved sexual activity, hypogonadal symptoms, and sexual desire in men who started with low libido. A separate narrative review of evidence in men 50 and older (human clinical review, PubMed/Embase/Cochrane search through 2025) points to supportive associations between TRT and improvements in bone density, lean body mass, and quality of life when therapy is physician-supervised.

Addressing the Cardiovascular Question Head-On

For years, testosterone therapy carried an FDA-mandated warning about possible cardiovascular risk, based on smaller, lower-quality studies. TRAVERSE was designed specifically to answer that question at scale. Across a mean follow-up of roughly 33 months, the primary cardiovascular endpoint (cardiovascular death, non-fatal heart attack, or non-fatal stroke) occurred in 7.0% of the testosterone group versus 7.3% of the placebo group — meeting the trial's pre-specified criteria for non-inferiority. TRAVERSE also found no increased incidence of high-grade prostate cancer or worsening urinary symptoms compared with placebo.

No study is without limits — TRAVERSE did note an increased risk of atrial fibrillation and, consistent with prior meta-analyses, an elevated risk of erythrocytosis (elevated red blood cell count) in the TRT group, which is exactly why lab monitoring during therapy matters.
